two factor authentication isn't working correctly

My iPhone 6s Plus with iOS10 Keeps wanting 2 factor authentication..for itself...within safari. So it asks for the code and then pops up with the code on the same device..this is clearly wrong lol, how can I fix this without disabling 2 factor all together? I no longer even see "trusted devices" as a thing anymore as well.
